Contest Family Meals Registration Form
(Update for 2023 State Convention)

Use this form to register Contest Winners and their Families (e.g. Drug Awareness, Americanism, Scholarship, Hoop Shoot, Soccer Shoot) for the banquet on Saturday, April 30, 2022.

Please submit this form no later than April 1, 2022. A separate form will be needed for each guest family.

The Lodge Sponsoring a Student Winner is responsible for the meal expenses of the Student Winner’s Family. The Student Winner’s meal is provided complimentary by the State Association

All meals include Mashed Potatoes, Vegetable, Salad, Rolls & Butter, and Dessert.

When you submit this form, a copy will be sent to your Lodge Contact’s e-mail for verification. Please send a check in the amount shown on the form total to:
